Dr. David Barile
Managing Partner & Medical Director
Dr. Barile is a leading geriatrician and thought leader in end of life care in New Jersey and nationally. He is board certified in internal medicine, geriatrics and hospice/palliative medicine, and serves as the Medical Director of University Medical Center of Princeton’s ACE unit (Acute Care for the Elderly) and Palliative Medicine Services.
Dr. Barile graduated from Eastern Virginia Medical School, completed his internship and residency at Beth Israel Medical Center in New York City and his geriatric fellowship at Mount Sinai School of Medicine. Dr. Barile is also the Founder and Executive Director of New Jersey Goals of Care, a leading advocacy group, and serves on the New Jersey Governors End of Life Advisory Council.
Mr. Hofmann has over 30 years experience in private equity investing. He was a Senior Partner of JP Morgan Partners and its predecessor Chase Capital, and Founder of Crystal Ridge Partners. His health care investments include Plus One, a leading provider of corporate fitness and wellness services, which was sold to United Healthcare in 2013.
Mr. Hofmann also serves as Chairman of a leading healthcare IT provider, and as an Advisor to Healthcare Recovery Solutions and serves on the Board of Realytics. He holds an MBA from Harvard Business School and a BBA from Hofstra University.
Dr. Randall S. Krakauer
Dr. Krakauer served as Aetna’s national medical director for Medicare for 12 years, and is a nationally recognized expert in end of life care. Dr. Krakauer has more than 30 years of experience in medicine and medical management, has held senior medical management positions in several major organizations. He is past chairman of the American College of Managed Care Medicine, and serves on the board of CTAC, the leading end of life advocacy group.
Dr. Krakauer is Board Certified in Internal Medicine and Rheumatology. He received training in Internal Medicine at the University of Minnesota Hospitals, in Rheumatology at the National Institutes of Health and Massachusetts General Hospital/Harvard Medical School, and received an MBA from Rutgers.
Ms. Miranda is a Certified Oncology Social Worker with over 25 years experience working with the terminally ill and hospice programs. She received her MSW from Washington University in St.Louis. She has devoted her career to assisting patients and families realize the importance of talking about the difficult subject of end of life.
Mr. Sylvestre has over 20 years experience forming and leading software product development organizations. He has driven three companies to successful exits, including an employee wellness platform acquired by United Healthcare – Optum, where he served as CIO for Optum Wellness. Mr. Sylestre received his master’s in Management Information Systems from Bentley University and his bachelors in CIS from Bryant University.
Director of Operations
Ms. Powsner has a private psychotherapy practice and works as a consultant with families wishing to Age in Place. She has worked in hospital and hospice settings for over 25 years and has extensive experience working with people facing serious illness and those in their final years of life. She received her Masters in Social Work from Boston University and her Bachelor's in Psychology from Antioch College. Ms. Powsner joined Your CarePlan in 2016 and oversees operations at all sites.
